The Non-Surgical Face Lift - Thread Lift

Thread Lifting is fast gaining popularity as a lunch time quick fix, as it helps patients to create a tighter and sharper V shape contour face without the downtime of a surgical facelift. 

 

If you wish to achieve a mini facelift or contouring but don’t want to go under the knife, you can choose to have an absorbable thread lift procedure instead.

 

The good news: It’s minimally invasive, with little to no downtime, and more affordable than surgery.

 

Currently there are only two types of thread lift material approved by HSA in Singapore, which are the polydioxanone (PDO) threads from Korea and the Silhouette soft (PLLA) from USA.

The Non-Surgical Face Lift - Thread Lift

PDO THREAD LIFT

PDO threads, originating from Korea, are the rage in Asia. There are numerous brands of PDO threads available in the market. PDO is a biodegradable synthetic polymer that has been used in surgical and suture (stitch) applications for many years and is safe for use in the human body.

These threads are manufactured through various processes, to create barbs or cogs that serve as a suspension in order to achieve tissue (fat and skin) repositioning.

 

About 6 months after the procedure, natural body processes will dissolve the PDO threads through simple hydrolysis. The lifted facial contours will remain for about 3-6 months more, due to the cellular rejuvenation effects. New collagen and blood vessels are stimulated and produced around the threads. At the same time, the PDO threads cause the fat tissue to contract. The overall results include skin tightening, fat repositioning and facial slimming effect, resulting in a V-shaped face.

PLLA THREAD LIFT

Poly L-lactic acid (PLLA) sutures by Silhouette Soft are US FDA-cleared and European CE-marked. These sutures consist of bidirectional cones made from a biocompatible polymer. This material has been used in orthopedic pins, rods, screws and nails for bone fractures, and of course, sutures, for many years.

PLLA sutures have greater longevity than PDO, averaging 12 to 18 months, and are eventually resorbed by the body.

The cones of the Silhoutte Soft thread catch on sagging fat and provide strong anchoring and repositioning of the sagging fat. Over a longer duration, the PLLA thread also stimulates collagen production for further skin tightening.

Both PLLA and PDO thread lift tighten sagging skin and stimulate collagen production through the insertion of these threads into the subcutaneous (fat) layer of the tissue using minimally invasive techniques.

WHAT ARE THE KEY DIFFERENCES BETWEEN

PDO THREADLIFT VS SILHOUETTE SOFT THREADLIFT?

  1. PDO Thread lift has superior immediate soft tissue lifting. The instant and immediate lift is often more prominent tighter as compared to Silhoutte Soft thread lift.

  2. PDO Threadlifts also provide superior lift for nose bridge augmentation. The Koreans are the inventors of the HIKO Nose thread lift, which is a PDO thread.

  3. Silhouette Soft thread lift has superior delayed collagen stimulation. The cones also give good lifting of sagging fat and compress the soft tissue together nicely.

 

In Astute Medical Aesthetics, we usually utilize both PDO and PLLA threads as a combination technique to achieve the maximum lifting and collagen stimulating  effect.

Frequently asked questions

How is the procedure performed?


A detailed discussion with the patient to explore individual concerns, treatment goals, and suitability of treatment is done first.

  1. Numbing cream will be applied over the face for 30 minutes.
  2. Areas of sagging skin and fat are identified and marked out as part of the thread-lift planning.
  3. The vectors (direction) and intended placement location of the threads are marked out on the skin with a marker. (The number of threads to be inserted varies among individuals and personalized assessment is required).
  4. Cleansing of the skin with antibacterial and antiseptic wash.
  5. Local anesthetic injections are made to the entry points. Nerve blocks may be performed to further enhance the numbing effect.
  6. The threads will then be inserted under the skin via needle entry points. The procedure is performed under sterile and clean technique. This will take between 30-60 mins depending on the complexity and number of threads inserted.
  7. The threads will then be tightened and any excess threads cut off. Any final adjustments will also be made at this point in time.
  8. Post procedure care and precautions will be explained and follow up appointments will be scheduled for review and follow up.

Patients are advised not to undergo any facials for a month and to avoid aggressive manipulation or massage of the face. It is also advised not to undergo any heat treatments such as radiofrequency or ultrasound devices for at least 3 months as this may damage the threads and result in thread breakage.




What to expect?


TIMELINE :

Immediate:

Your skin and soft tissue is immediately lifted as the threads repositions soft tissue into its new position. There will be some swelling immediately after the procedure and for the next few days, while you will also experience some tightness of the face.

1 week later:

Most of the swelling will have subsided and all the entry points used for the insertion of the threads would have healed. You will begin to look and feel more natural.

3 months:

Skin rejuvenation from collagen stimulation and production peaks. You will notice improved skin texture, smoother and firmer skin.

6 months:

The PDO threads would start to dissolve now, but the results will persists for several more months as a result of the collagen production.

Recommendation:

Touch ups 6 months after the initial treatment, to allow for a long lasting result of up to 2 years.




What are the risks / downtime associated with PDO, PLLA Threadlifts?


As with all medical procedures, there are possible risks and complications that you need to be aware of. The most common adverse effects encountered are temporary and include swelling, bruising and pain. These usually resolve within one week and constitute much of the downtime of the procedure. Some patients may experience mild skin irregularities and dimpling which will resolve with time. Less frequently, patients may have prolonged aching or soreness that will take several weeks to resolve completely. Other less common complications include thread breakage, extrusion, infection or damage to underlying nerves and vessels. Meticulous care ensuring that the threads are placed in the correct tissue depth and planes, as well as sterility during the thread lift procedure will minimize these risks. In the unlikely event of thread extrusion or infection, the offending thread will need to be removed under local anesthesia and antibiotics will be needed.





Combination treatments

It is important to understand the process and anatomy of aging in order to achieve the best results. As we age, all tissue layers from the skin to the bone undergo changes.

  1. The skin loses collagen and elastin, resulting in skin laxity.

  2. Superficial (subcutaneous) fat increases in volume and droops, causing nasolabial lines and marionette lines to become more prominent.

  3. The muscle layers (SMAS) also become increasing lax.

  4. Deep fat loses volume and bone is resorbed, resulting in reduced deep tissue support.

In order to achieve the best and long lasting results from thread lifting, we have to identify the key issues in each individual in order to tailor an optimal treatment plan.

For individuals with significant volume loss and lack of structural support, it is necessary to restore the lost volume with fillers first before undergoing thread lifting.

It is also recommended to combine thread lifting with High Intensity Focused Ultrasound (HIFU) devices as this treatment can target the SMAS layer for tightening, and the heat generated from the device can also help in reducing some of the excess subcutaneous fat.

In general, the treatment plan will begin with HIFU devices, followed by volumizing fillers and lastly thread lifting.
